Only show list of categories on user page if "assigned categories only" is checked

Some people are checking all of the categories on a user page, assuming that's necessary in order for the user to have access to all categories. In reality the categories only have an effect when the "assigned categories only" checkbox is checked.

To make this page clearer we could hide the list of categories until the "assigned categories only" checkbox is checked.
