Helios Airways Flight 522 was a scheduled passenger flight from Larnaca, Cyprus to Prague, Czech Republic with a stopover to Athens, Greece, that crashed on 14 August 2005, killing all 121 passengers and crew on board. A loss of cabin pressurization incapacitated the crew, leaving the aircraft flying on autopilot until it ran out of fuel and crashed near Grammatiko, Greece. It was the deadliest aviation accident in Greek history. - Wikipedia


The crew had cabin pressurization control panel set in 'MAN" (MANual) so the aircraft was not regulating it's environment to vent out CO2, because they misidentified altitude warning alarm as takeoff configuration warning and they fell uncontinous before being able to fix the issue causing the aircraft to fly on autopilot until it ran out of fuel and crashed.

Remedy of this was to have a separate annunciator for the pressurization warning.

Experimenting with status reporting should be done system-wise in our design.
In case of loss of power we should consider hardware identification.
