Add custom fields to Test Cases and Test Plan

We have our own test case template and not all of our columns are in Kiwi TCMS.
We would like to ask how we can add custom fields (test case and test plan).
Not sure if this is doable, but please help to coordinate with us.
Columns to add:

1. Test Data
2. Pre-requrements
3. Post-requirements
4. Expected Results
5. etc. see attached

And if it is possible to rename the field name based on our existing document?
