Second parent's address is enabled only if checkbox is checked

Bussiness context:
Because many children have 2 parents, and the common thing is that they have the same address we want to:
AC:

• have a checkbox with label: "Adres inny niż u pierwszego rodzica",
• if checkbox is check we user can insert second parent's address.
• if checkbox is not checked second parent address inputs's are disabled.
